Assisted living
Help with the everyday things — bathing, dressing, medication management, meals. Independence preserved where it matters most.
Ask about assisted livingA warm, community-first assisted living home in Nephi. Halloween Bashes and holiday parties. Family visits over homemade cookies. The kind of care where Fred's story is famous — and yours will be too.
Every resident's needs shift over time. Autumn Park's team assesses continuously and adapts care levels — always with the family in the loop, always with dignity first.

Ask about memory careShort-term stays for families needing a break, or after a hospital discharge. Nights, weekends, or weeks — flexible arrangements.
Ask about respiteFrom the Halloween Bash (thank you Lily Twitchell!) to Fred's famous Halloweekend costume — Autumn Park's events are the real deal. Real families, real neighbors, real fun.

Fred definitely enjoyed his Halloweekend.
Fred usually steals the show at costume time. Cookie decorating, games, and families welcome — it's the kind of night that makes the whole community feel like home.

The best Halloween Bash of 2023 — thanks to friends and family for coming.
The Halloween Bash is an annual tradition every October 31st. Residents, staff, neighbors, and family fill the halls. Costumes encouraged.

Bring the family. Homemade meals, live music sometimes, always a full room.
One evening every month we set the tables long and open the doors. Bring the grandkids. Bring a neighbor. There's always another chair.

Trips to local diners, the town square, community events.
Wheelchair accessible transport. Residents pick where they want to go — and we go. Small town, big life.
Not a program schedule. Not a facility calendar. Just the everyday and the seasonal, the small rituals and the big parties that turn a building into a community.

You'll meet the staff, see the common areas, and (if a resident is comfortable) meet a few neighbors. Tell us about your loved one's situation and we'll follow up with tour times that work for your family.
